I started this page in April, 2013. I will use it to develop evocative, possibly even provocative arguments.

I am experimenting with embedded video as a means of transmitting the message, co-opting some of the presentation ideas I use in my workshops.

Many of the arguments I plan for this page are in response to rather common attitudes that I view as misguided.

Why do many organizations have a dysfunctional safety culture?  Why do some have a healthy culture?  In this brief, I discuss how a dysfunctional culture can be transformed into a healthy one. 

 

 

 

 

 

I review some of the issues surrounding cognitive error and briefly discuss ideas for reducing their frequency and for dealing with their consequences.
